Введение. Рак молочной железы (РМЖ) – редкое онкологическое заболевание, составляющее менее 1% всех случаев злокачественных новообразований молочной железы и менее 0,5% от общей структуры онкологической патологии у мужчин. РМЖ у мужчин является серьезной лечебно-диагностической проблемой. Установление диагноза РМЖ на ранних стадиях затруднительно, проводится дифференциальная диагностика прежде всего с гинекомастией с гиперплазией железистых протоков. Диагноз РМЖ у мужчин в далеко зашедших стадиях не представляет затруднений. В диагностике РМЖ у мужчин используются рентгеномаммография, ультразвуковое исследование, соге-биопсия с гистологическим и иммуногистохимическим исследованием материала. Основным методом лечения при первично операбельной и местнораспространенной форме РМЖ является хирургический. Тактика лечения определяется стадией заболевания, молекулярно-биологическим подтипом опухоли, возрастом пациента и сопутствующей патологией. Цель исследования. На примере клинического случая РМЖ у мужчины оценить современное состояние диагностики и лечения данной патологии. Материал и методы. В работе представлен клинический случай РМЖ у мужчины. Результаты и их обсуждение. Приведенный клинический случай РМЖ у мужчины демонстрирует важность мультимодального подхода (визуализация, гистологическое исследование, ИГХ исследование) для верификации диагноза и выборе правильной тактики лечения. Выводы. На основании анализа современной литературы и приведенного клинического случая для практического применения целесообразно: – Внедрение скрининговых УЗИ-программ для мужчин с гинекомастией. – Обязательное использование шкалы BI-RADS для стандартизации отчетов. – Молекулярное исследование всех случаев РМЖ у мужчин. – Информирование пациентов мужского пола о необходимости обращения за помощью при любых жалобах со стороны молочной железы, особенно обнаружении уплотнения в ней.
Introduction. Breast cancer (BC) is a rare oncological disease that accounts for less than 1% of all cases of breast malignancies and less than 0.5% of the total structure of oncological pathology in men. Breast cancer in men is a serious medical and diagnostic problem. The diagnosis of breast cancer in the early stages is difficult, differential diagnosis is carried out primarily with gynecomastia with glandular duct hyperplasia. The diagnosis of breast cancer in men in advanced stages is not difficult. X-ray mammography, ultrasound, and biopsy with histological and immunohistochemical examination of the material are used in the diagnosis of breast cancer in men. The main treatment method for the primary operable and locally advanced form of breast cancer is surgical. Treatment tactics are determined by the stage of the disease, the molecular biological subtype of the tumor, the patient's age and concomitant pathology. The purpose of the study. To evaluate the current state of diagnosis and treatment of this pathology using the example of a clinical case of breast cancer in a man. Materials and methods. The paper presents a clinical case of breast cancer in a man. The results and their discussion. The above clinical case of breast cancer in a man demonstrates the importance of a multimodal approach (visualization, histological examination, IHC examination) for verifying the diagnosis and choosing the right treatment tactics. Conclusions. Based on the analysis of modern literature and the given clinical case for practical application, it is advisable: – Introduction of screening ultrasound programs for men with gynecomastia. – Mandatory use of the BI-RADS scale for standardization of reports. – A molecular study of all cases of breast cancer in men. – Informing male patients about the need to seek help for any breast complaints, especially if a lump is found in it.
