The article substantiates the relevance of the development of an integrated model of professional formation of students. The conceptual representation of the phenomenon is given as a process of actualization of professional and personal potential of the individual, as the first stage of professional formation. The indexes and indicators of this process are proposed, indicating the current and milestone potential value. Their meaning and role in the assessment of professional formation is revealed. The methods of their evaluation and the results of empirical studies of these indexes for some of the stages of professional development of pupils, students and graduates are presented. Obtained by different methods values of the indexes allow us to explain some of the causes of dysfunction, such as socio-psychological process as professional formation of youth. The importance of monitoring this process by means of evaluating appropriate indexes is shown.
